We are seeking a highly skilled Pediatric Hi-Tech Registered Nurse to join our team at VNA & Hospice of the Southwest Region Inc.

About the Role

This is a per diem weekend position, providing home health care services to pediatric patients in Rutland County. As a Pediatric Hi-Tech Registered Nurse, you will play a key role in delivering high-quality, patient-centered care in the comfort of patients' homes.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of patients' needs, develop individualized care plans, and communicate with patients and caregivers to ensure effective care delivery.
  • Care Coordination: Coordinate with healthcare teams, including physicians and other healthcare professionals, to ensure seamless care transitions and optimal patient outcomes.
  • Patient Education: Provide education to patients and caregivers on medication management, treatment options, and self-care techniques to promote informed decision-making and improved health outcomes.
  • Documentation and Reporting: Maintain accurate and up-to-date documentation of patient care, including progress notes, medication records, and other relevant information.
  • Quality Improvement: Participate in quality improvement initiatives to enhance patient care and services, and contribute to the development of policies and procedures to ensure compliance with regulatory requirements.
Requirements
  • Education: Graduation from an accredited school of nursing, with a Bachelor's degree in Nursing preferred.
  • Experience: Prior clinical acute care nursing experience, with a focus on pediatric care.
  • Licensure: Valid VT Registered Nurse license, with a valid Driver's License required.
