Key Responsibilities:
- Attend to personal care needs as assigned and respond promptly to patient requests.
- Communicate any inability to perform specific tasks to the Case Manager immediately.
- Utilize positive communication techniques with patients and families, ensuring ethical and respectful care, including maintaining confidentiality.
- Ensure patient safety by using equipment correctly (e.g., foot stools, side rails, O2).
- Provide personal care such as bathing, back rubs, oral hygiene, and changing bed linens as needed.
- Assist patients with dressing and undressing as required.
- Plan and prepare nutritious meals, including shopping, as assigned.
- Aid in feeding patients as necessary.
- Measure and record vital signs such as temperature, pulse, respiration, and blood pressure as ordered, demonstrating competency in these skills.
- Monitor and care for the patient's skin to prevent tissue breakdown.
- Report any significant changes in the patient's condition to the Case Manager.
- Be aware of interpersonal issues involving caregivers or others living with the patient.
- Assist with ambulation and exercise as directed by the hospice nurse or therapist.
- Provide normal range of motion exercises and assist with positioning as instructed.
- Help with self-administered medications as permitted by state regulations, such as reminders.
- Offer assistance with bedpans and urinals as needed.
- Support light laundry tasks as assigned.
- Perform range of motion and other simple procedures as part of therapy services as ordered, demonstrating appropriate skills competency.
- Provide respite for patients, families, or caregivers when on-site, as appropriate.
- Maintain cleanliness and orderliness in the patient's living area as assigned.
- Follow the organization's documentation and care standards, adhering to professional conduct.
- Participate in quality assessment and performance improvement initiatives.
- Show respect for the patient and family environment and the personal needs of the patient.
Qualifications:
- Must meet state training requirements.
- Completed a minimum of seventy-five hours of classroom and supervised practical training, with sixteen hours of classroom training prior to a mandatory sixteen hours of practical training.
- At least six months of experience in providing personal care.
- Minimum age of eighteen years.
- Ability to read and follow written instructions and document care provided.
- Understanding of hospice philosophy and comfort in providing specialized care to terminally ill patients.
- Satisfactory references from previous employers.
- Capability to work with minimal supervision.
- Comfort with issues surrounding death and dying.
- Maintain a calm demeanor when in a patient's home.
- Possess and maintain current CPR certification.
- Exhibit tact, patience, and good personal hygiene.
- Successfully complete a probationary period to ensure competency in the above qualifications, documented by the supervisor and the employee.
- Strong oral and written communication skills.
- Good organizational abilities.
