Job Summary

Activa Home Health is seeking an experienced and licensed Certified Occupational Therapy Assistant (COTA) to join our team as a Field Occupational Therapy Assistant. As a key member of our multidisciplinary team, you will provide medically prescribed, goal-oriented therapy to patients in their homes, working closely with physicians, therapists, and other healthcare professionals to ensure the highest quality of care.

Responsibilities
  • Participate in patient plan of care under the supervision of the Occupational Therapist, making visits to patients and communicating with physicians as needed.
  • Follow the Plan of Care and document care and submit electronic documentation in an accurate and timely manner to update and maintain patient medical records.
  • Provide education and instruction to team members, patients, and their families, and participate in regularly scheduled interdisciplinary team meetings to coordinate care and exchange information.
  • Apply agency policies and procedures, as well as Federal and State Regulatory agency rules and regulations, in providing care.
  • Coordinate patient/family plan of care and ensure the care plan is reviewed at every team meeting, assessing patient's level of care, needs, and requirements.
Requirements
  • Currently licensed as a Certified Occupational Therapy Assistant (COTA) in this state and in good standing with the Board in which he/she will practice.
  • Knowledge of principles, concepts, and methodology of physical therapy.
  • Ability to work well with an interdisciplinary team, develop and maintain rapport with patients and families, and demonstrate problem-solving and decision-making abilities.
  • Excellent written and verbal communication skills.
