We are seeking a skilled Physical Therapist to join our patient care team at Stat Home Health. As a Physical Therapist, you will play a vital role in evaluating patients' functional status and developing personalized plans of care.

Key Responsibilities:

  • Evaluate patients' physical therapy needs and develop plans of care in collaboration with physicians and healthcare teams.
  • Provide high-quality physical therapy services within the scope of practice, adhering to state laws and regulations.
  • Monitor patients' progress, reporting any changes or concerns to physicians and supervising registered nurses.
  • Prepare written summaries for physicians at regular intervals.
  • Set rehabilitation goals for patients and guide them in using therapeutic activities to improve function.

Requirements:

  • Current licensure by the Florida Physical Therapy Board.
  • At least one year of experience in Home Health Care as a Physical Therapist.
  • CPR certification.
  • Excellent time management skills and a patient-centered approach.
  • Reliable transportation for patient visits.
