Healthcare Support Specialist

We are seeking a skilled and compassionate Healthcare Support Specialist to join our team at Central City Concern. As a member of our primary care team, you will play a vital role in delivering comprehensive, client-centered, and trauma-informed integrated services to our patients.
Key Responsibilities- Work collaboratively with our multi-disciplinary team to provide integrated primary care services to homeless and low-income patients.
- Proactively identify opportunities for improvement within our workflows and logistics, and participate in review of key performance indicators.
- Build relationships with patients and support their connection to culturally responsive and culturally specific services.
- Receive, document, and direct patient telephone calls in a timely and complete manner.
- Prepare prescription refills under provider orders, and electronically file and index clinical information into patient charts.
- Provide follow-up contact to patients identified by care team members for support and management of health-related issues.
- Participate in outreach activities to offer services and engage participation in a non-coercive manner.
- High school diploma or GED required; prefer Bachelor's Degree or higher in health or social service-related field of study.
- At least one year of experience working with homeless, low-income, or other vulnerable populations in a supportive role preferred.
- Ability to communicate effectively orally and in writing, and to understand healthcare-related terminology.
- Ability to work in an environment where people may be hostile or abusive, and to maintain accurate records and complete necessary paperwork.
