Healthcare Coordinator

Join our team at Meridian Illinois as a Healthcare Coordinator, where you will play a vital role in transforming the health of our communities. As a key member of our team, you will be responsible for developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes.
Key Responsibilities:- Evaluate the needs of our members, identify resources, and recommend plans for the best outcomes.
- Develop and implement ongoing long-term care plans, working with providers, specialists, and community resources to ensure members receive the necessary services.
- Coordinate between members, families, caregivers, and care providers to ensure identified services are accessible.
- Provide resource support to members and their families for various needs, such as employment, housing, and independent living.
- Monitor care plans, member status, and outcomes, making recommendations for improvement.
- Interact with long-term care healthcare providers and partners to ensure member needs are met.
- Collect, document, and maintain member information and care management activities to ensure compliance with regulations.
- Perform on-site visits to assess member needs and collaborate with providers or resources as needed.
- Provide education to members and their families on procedures, healthcare provider instructions, service options, referrals, and healthcare benefits.
- Offer feedback to leadership on opportunities to improve and enhance quality of care and service delivery.
- Bachelor's degree in a related field.
- 1 year of related experience.
- Must meet one of the following criteria: RN licensed in Illinois, Bachelor or Master's degree in a human services-related field, or LPN with 1 year of experience in conducting comprehensive assessments and provision of formal service for the elderly.
