ResponsibilitiesThe Clinical Triage Nurse Practitioner will serve as the primary provider and clinical point of contact for patients requiring evaluation for issues that may necessitate further clinical intervention.
Key responsibilities include:
- Acting as a clinical resource for triage teams to address patient needs
- Evaluating and treating patients via telehealth to address acute needs
- Coordinating care with local clinical teams to ensure comprehensive patient care
- Addressing time-sensitive medication refills, lab results, and other patient needs
- Interacting with primary care providers to escalate or evaluate patient needs
Required qualifications include:
- Certified Nurse Practitioner with active licenses in coverage states
- Active DEA license
- Excellent written and verbal communication skills
- Ability to react decisively in urgent situations
Preferred qualifications include:
- 1 year of experience at HarmonyCares Medical Group
- Experience with APRIMA EMR
- Experience with telehealth and coordinating care virtually
