Job Summary

Deaconess Cross Pointe is seeking a skilled Family Nurse Practitioner to join our experienced team of healthcare professionals. This supplemental position does not require Mental Health Certification or specialization. The successful candidate will work closely with other NPs to complete History & Physicals on newly admitted patients within 24 hours of admission.

Key Responsibilities
  • Plan and coordinate patient discharge and/or transfer.
  • Respond to the Emergency Department for evaluation and treatment of patients.
  • Obtain histories and perform physical examinations.
  • Diagnose and manage acute and chronic health conditions via evidence-based guidelines.
  • Order, conduct, and interpret diagnostic studies and laboratory tests.
  • Perform diagnostic and therapeutic procedures, including suturing, wound care, incision and drainage, splinting, casting, biopsies, and invasive procedures.
  • Prescribe or recommend pharmacologic and non-pharmacologic therapies, including controlled medications.
  • Consult with physicians for patient care needs.
  • Refer patients to appropriate sources of care as indicated.
  • Work to exceed national benchmarks for quality care delivery, cost containment, and length of stay for various disease states.
  • Assist patients to maximize their abilities to cope with health and illness.
  • Present and document patient data.
  • Collaborate with the hospitalist team to reduce patient length of stay by performing tasks and evaluations in a timely manner.
  • Ensure the efficient utilization of resources while supporting clinical pathway implementation and compliance.
  • Ensure appropriate and timely referrals to an appropriate facility as indicated by the patient care needs.
  • Participate in the assurance of robust communication systems, including telephone communications, on admission/status updates, discharge summaries, etc.
  • Support the primary care physician/patient relationship.
  • Actively establish primary care relationships for patients who do not have primary care physicians.
  • Maintain a collegial relationship with the interprofessional team involved in care of the patient, inclusive of the primary care physician.
  • Take responsibility for assuring patient/family satisfaction with care provided.
  • Promote self-care, health promotion, and disease prevention through the provision of information, counseling, and education for individuals, families, and groups as indicated.
  • Develop and participate in community outreach programs/activities.

Weekday and weekend after-hour on-call requirements. No holidays.

Additional Responsibilities
  • Assist with relevant billing documentation and development of efficient billing processes.
