Position Overview:

The Senior Care Management Nurse serves as a vital resource and advocate for families caring for elderly relatives or individuals with disabilities.   11. Connect clients and families with essential medical, legal, housing, insurance, and financial resources.
  12. Guide clients in managing their health and wellness.
  13. Complete necessary documentation promptly.
  14. Identify and address gaps in care through strategic analysis.
  15. Assist in determining appropriate services, including home health and hospice care.
  16. Review financial matters and consult with relevant professionals as needed.
  17. Support clients and families with insurance inquiries and claims.
  18. Identify local resources to meet client needs.
  19. Advocate effectively for clients, ensuring their preferences are respected in healthcare decisions.
  20. Refer clients to legal and financial experts when necessary.
  21. Provide crisis intervention and support during emergencies and hospitalizations.
  22. Conduct ongoing assessments to help clients achieve their highest functional potential.
  23. Encourage independence while addressing safety and security concerns.
  24. Perform additional duties as assigned by the Director of Care Management.

Required Knowledge and Skills:

Qualifications:

  • Nursing degree or equivalent experience.
  • Current driver's license and vehicle insurance.
  • Licensed Nurse with a minimum of one year of post-graduate case management experience.
  • Experience in complex care management and community-based services.
  • Proficiency in electronic medical records and computer applications.
  • Access to a secure computer and high-speed internet for timely data entry.
