The Senior Care Management Nurse will offer expert guidance to clients within their residences, ensuring comprehensive care planning is conducted weekly.
Key Responsibilities:
- Conduct in-home visits to assess client needs.
- Develop a detailed care plan based on thorough evaluations of physical, functional, and psychosocial conditions.
- Facilitate hospital or facility visits as necessary.
- Coordinate communication between clients and their families to clarify treatment strategies.
- Attend medical appointments to enhance communication among healthcare providers, clients, and families.
- Engage actively with the healthcare system to ensure client needs are met.
- Oversee medication management and reconciliation.
- Evaluate the client’s living environment and overall well-being.
- Empower clients to maintain independence and thrive at home.
- Assist families in navigating caregiving challenges, whether local or long-distance.
- Connect clients and families with essential medical, legal, housing, insurance, and financial resources.
- Guide clients in managing their health and wellness.
- Complete necessary documentation promptly.
- Identify and address gaps in care through strategic analysis.
- Assist in determining appropriate services, including home health and hospice care.
- Review financial matters and consult with relevant professionals as needed.
- Support clients and families with insurance inquiries and claims.
- Identify local resources to meet client needs.
- Advocate effectively for clients, ensuring their preferences are respected in healthcare decisions.
- Refer clients to legal and financial experts when necessary.
- Provide crisis intervention and support during emergencies and hospitalizations.
- Conduct ongoing assessments to help clients achieve their highest functional potential.
- Encourage independence while addressing safety and security concerns.
- Perform additional duties as assigned by the Director of Care Management.

- Access to a secure computer and high-speed internet for timely data entry.
