Job Summary

We are seeking a compassionate and skilled Nurse Aide/HHA to join our team at Concordia of Franklin Park. As a Nurse Aide/HHA, you will play a vital role in providing direct resident care related to Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).

Responsibilities
  • Assist residents with ADLs and IADLs, including eating and drinking, ambulating, transferring, toileting, bladder and bowel management, personal hygiene, bathing, dressing, turning and positioning, and documenting care provided.
  • Assist residents with medication administration, including identifying correct residents, measuring vital signs, removing medication from original containers, crushing or splitting medication as ordered, and completing documentation.
  • Respond to resident call bells in a timely manner, prioritizing resident needs and addressing concerns.
  • Communicate important resident observations, changes in condition, findings, and problems to the Charge Nurse in a timely and proactive manner.
  • Commit to high ethical standards, complying with the Code of Conduct, state and federal regulations, accreditation standards, Concordia policies, procedures, and safety standards, and HIPAA rules for maintaining confidentiality of resident/employee records and information.
Requirements
  • Caring, compassionate, and understanding attitude.
  • High school diploma, GED diploma, or active registry status on the Pennsylvania nurse aide registry.
  • 18 years of age or older.
  • Free from a medical condition, including drug or alcohol addiction, that would limit the staff person from providing necessary personal care services with reasonable skill and safety.
