We are seeking a skilled Physician to join our team as a Primary Care Specialist. As a member of our team, you will provide primary health care and patient care management in a residential setting, working closely with our patients and their families to ensure the best possible outcomes.

Responsibilities
  • Deliver primary health care and patient care management in a residential setting
  • Provide high-quality, coordinated care to patients with complex medical needs
  • Work collaboratively with our interdisciplinary team to ensure seamless care transitions
  • Develop and implement individualized care plans to meet the unique needs of each patient
Qualifications
  • Experience in Geriatrics or complex patient care preferred
  • Active and unrestricted medical license and DEA
  • Board Certified or Board Eligible
  • EMR experience
