About the Role

Clover Senior Care and Home Health Care is seeking a skilled Speech Therapist to join our team. As a Speech Therapist, you will work with patients in the greater Indianapolis area, providing high-quality speech therapy services in the comfort of their own homes.

Responsibilities
  • Conduct patient evaluations and assessments to identify speech and language disorders
  • Develop and implement individualized treatment plans to address patient needs
  • Provide speech therapy services to patients, including exercises and activities to improve communication skills
  • Document patient progress and maintain accurate records
  • Collaborate with other healthcare professionals to ensure comprehensive patient care
Requirements
  • Current Speech Therapy License in the state of Indiana
  • Valid drivers license, valid auto insurance, and reliable transportation
  • Current CPR/In-Person Certification
  • Valid TB test (past month) or have one placed in our office
  • Ability to work independently and as part of a team
Preferred Qualifications
  • Proficient in using EMR systems, including AXXESS
  • Minimum of 2 years Home Health experience
  • General knowledge of home health practices, including familiarity with relevant state and local services
