Job Summary:

The LTSS Service Coordinator-RN Clinician will be responsible for managing the overall care of our members, including developing and implementing individualized care plans, coordinating with healthcare providers, and ensuring seamless transitions between care settings. This role requires a strong clinical background, excellent communication skills, and a passion for delivering high-quality patient care.

Key Responsibilities:
  • Develop and implement individualized care plans for members, taking into account their unique needs and goals.
  • Coordinate with healthcare providers, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care.
  • Conduct thorough assessments of members' physical and behavioral health needs, identifying areas for improvement and developing strategies for optimal care.
  • Collaborate with members, caregivers, and family members to ensure they are informed and engaged in the care planning process.
  • Manage and coordinate care transitions, ensuring seamless transitions between care settings.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
Requirements:
  • Current, unrestricted RN license in applicable state(s) required.
  • Minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
  • Strong clinical background and excellent communication skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
Preferred Qualifications:
  • MA/MS in Health/Nursing preferred.
  • Experience working in a healthcare setting, preferably in a case management or service coordination role.
