The ideal candidate will possess a strong understanding of family-driven care and be passionate about serving youth with complex needs. As a Wraparound Facilitator, you will facilitate a high-fidelity wraparound process, working closely with families, providers, and social support networks to develop individualized plans of care that meet the unique needs of each youth.

Key responsibilities include:

  • Collaborating with families and key stakeholders to build comprehensive plans of care that address behavioral and mental health challenges.
  • Working with Independent Evaluators to ensure thorough assessments and effective treatment planning.
  • Assisting youth and families in accessing strength-based mental health, social services, educational services, and other necessary resources.

To excel in this role, you must have:

  • A Bachelor's degree in a relevant field (social sciences, human services, or behavioral sciences) or equivalent experience.
  • Previous experience working in family, children/youth, or community service capacities.
  • Excellent communication and collaboration skills, with the ability to work effectively with diverse stakeholders.
  • Capacity for extensive travel and driving, as well as 24/7 on-call crisis intervention.

We offer a competitive hourly rate of $17.00 and a range of benefits, including low-cost medical, dental, and vision insurance, paid time off, and retirement savings plan with employer match.
