Key Responsibilities

  • Administer personal care and related services to patients in their homes, as assigned by the Case Manager or Hospice Manager.
  • Work collaboratively with the healthcare team to develop and implement individualized care plans.
  • Provide emotional support and education to patients and their families, promoting independence and self-care.
  • Maintain accurate and detailed records of patient care, including progress notes and medication administration.

Requirements

  • Education: High school diploma or equivalent required.
  • Experience: Six months of experience working with people, with a strong interest in providing care and support.
  • Certifications: Nursing Assistant or Home Health Aide certification, American Heart Association Basic Life Support (BLS) required.
