The In-Home Nurse Practitioner at Abode Care Partners provides high-quality medical care to seniors and individuals with complex needs in various home settings. This role involves evidence-based medical care, collaboration with healthcare professionals, and management of patient medical records to enhance quality of life.

About the Role:

  • Provide personalized medical care to patients in their homes, assisted living facilities, or independent living communities.
  • Develop and implement comprehensive care plans in collaboration with caregivers and other healthcare professionals.
  • Perform routine check-ups, treatments, and exams to ensure optimal patient outcomes.
  • Order and interpret laboratory and diagnostic testing as needed.
  • Manage patient medical records and maintain accurate documentation.

Qualifications and Requirements:

  • A licensed nurse practitioner (NP) with current state license and national board certification.
  • Active DEA licensure and a Master's degree in Nursing (MSN).
  • Five or more years of nursing experience, with at least two years as a nurse practitioner.
  • Excellent interpersonal communication skills, both verbal and written.
  • Strong time management skills with accountability for daily workflow.
  • Proficiency in medical instruments and equipment, computer-based data management programs, EMR systems, and Microsoft Office applications.
  • A valid driver's license with automobile insurance.
