Job Title: Licensed Practical Nurse - Medication Care Manager

Sunrise Senior Living is seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a Medication Care Manager. As a Medication Care Manager, you will play a vital role in ensuring the highest quality of care for our residents by administering medications and treatments in a safe and organized manner.

Responsibilities:
  • Administer medications and treatments to residents in accordance with their individualized care plans.
  • Review and document medication administration records to ensure accuracy and compliance with regulatory requirements.
  • Collaborate with the Resident Care Director (RCD) and Wellness Nurse to ensure medication updates and changes are implemented promptly.
  • Conduct regular audits of medication carts and treatment areas to ensure compliance with safety protocols.
  • Participate in the development and implementation of Individualized Service Plans (ISPs) and Individualized Care Plans (ICPs) to ensure resident-centered care.
  • Provide education and training to residents and their families on medication management and other health-related topics.
  • Contribute to the overall engagement and satisfaction of residents, families, and team members by promoting a positive and supportive work environment.
Requirements:
  • Current LPN licensure in the state of employment.
  • Minimum of 1 year of experience in a long-term care or senior living setting.
  • Strong knowledge of medication administration and management principles.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
  • Commitment to providing high-quality care and services to residents and their families.
