Care Team Leader

About the Job:
We are looking for a skilled Nurse Practitioner to lead our long-term care team at Pebble Creek Nursing Center. As a Nurse Practitioner, you will be responsible for implementing resident care plans in compliance with Company and federal guidelines. You will also provide direction to Nurse Aides and ensure accurate and timely documentation of residents' charts.
Requirements:
- Registered Nurse or Licensed Vocational Nurse in good standing
- Excellent technical, assessment, and documentation skills
- Good communication skills
- Ability to organize workflow and respond to emergencies
