Nurse Care Specialist

Gentiva Hospice is a leading provider of hospice care, and we are seeking a compassionate and skilled Registered Nurse to join our team. As a Hospice RN, you will play a vital role in delivering high-quality patient care and providing emotional support to patients and their families.
Key ResponsibilitiesAs a Hospice RN, your key responsibilities will include:
- Providing direct patient care services to hospice patients and supportive care to the patient's family.
- Acting as a primary liaison between the physician, patient/family, and hospice team.
- Managing and collaborating with an interdisciplinary group to provide comfort, peace, and dignity to patients.
- Evaluating patient and family needs and assisting in the preparation and execution of the plan of care for each patient.
- Maintaining all assigned patient clinical records.
- Participating in company quality assessment and performance improvement programs.
To be successful in this role, you will need:
- A RN license in the state of requested employment (or eligibility to obtain).
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology.
- Additional experience in hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing is preferred.
- Current driver's license and automobile insurance.
- Current CPR certification.
