Job Summary

Advocate Aurora Health is seeking a skilled Registered Nurse (RN) to join our Home Health team as a Case Manager. This is a full-time position that requires a strong commitment to delivering high-quality patient care in the comfort of patients' own homes.

Key Responsibilities
  • Assess patients' needs and develop individualized care plans to ensure optimal health outcomes.
  • Provide comprehensive nursing care to patients, including wound care, medication management, and education on disease management.
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between healthcare settings.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Graduate from an accredited school of nursing.
  • One year of medical-surgical experience in an acute or subacute care facility.
  • Current Illinois RN license.
  • Current CPR certification.
  • Ability to lift up to 35 pounds without assistance.
  • Ability to assist with ambulation, including turning and transferring patients up to 200 pounds.
