Key Responsibilities
- Provide individual and group therapy sessions to seniors in sub-acute, long-term care, and assisted living communities
- Collaborate with the treatment team to develop and implement treatment plans
- Conduct initial assessments and follow-up sessions
- Document patient progress using our electronic medical/health system
- Maintain professional development and licensure requirements
Requirements
- Master's degree in Social Work or related field
- Licensed Clinical Social Worker (LCSW) in good standing
- Experience working with geriatric population preferred
- Excellent communication and interpersonal skills
