Clinical Supervisor Job Description

The Clinical Supervisor is a key member of our team, responsible for overseeing patient care activities within the home care setting. This role requires a strong clinical background, excellent communication skills, and the ability to work effectively with patients, families, and healthcare teams.

Key Responsibilities:
  • Coordinate care to assigned patients, establishing a goal-directed care plan from admission to discharge.
  • Perform on-site supervisory visits to assess patient, family, environment, and clinical caregivers.
  • Ensure timely follow-up documentation and maintain accurate records.
  • Uphold and promote adherence to CHAP standards, ensuring high-quality and compliant nursing services.
  • Contribute to retention initiatives by fostering a supportive work environment and promoting caregiver satisfaction.
  • Ensure availability and proper operation of necessary equipment and supplies related to patient care.
  • Assess patients and provide direct patient care as needed.
  • Manage expectations and satisfaction with internal and external customers.
  • Evaluate the quality and effectiveness of nursing services and analyze clinical data to improve patient outcomes.
Requirements:
  • Diploma, Associate, or Bachelor's degree in nursing from a state-accredited RN program.
  • Clinical or other work experience as per state and/or federal laws and regulations.
  • RN licensure in designated state(s) as appropriate.
  • Valid Driver's license and Acceptable MVR.
Preferred Qualifications:
  • RN experience and knowledge of effective private duty program administration.
Physical Requirements:
  • Ability to speak, write, read, and understand English.
  • Ability to travel; company does not provide vehicles or transportation.
  • Occasional lifting, carrying, pushing, and pulling of up to 40 pounds.
  • Prolonged walking, standing, bending, kneeling, reaching, twisting.
  • Ability to sit and climb stairs.
  • Must have visual and hearing acuity.
  • Must have a strong sense of smell and touch.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
  • Must be able to appropriately respond physically and mentally to emergency situations in the home or during transport.
Work Environment:

Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions.

Possible exposure to blood, bodily fluids, and infectious diseases.
