About the Role

Benchmark Human Services is seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health Care team. As an LPN, you will provide high-quality nursing care to patients in the comfort of their own homes.

Key Responsibilities
  • Provide professional nursing care to patients as defined in the nurse practice act and under the direction of a Registered Nurse (RN).
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Monitor patient progress, identify changes in condition, and report to the RN Clinical Care Manager and physician as needed.
  • Collaborate with the healthcare team to ensure timely and effective communication of patient information.
Requirements
  • Valid Indiana Nursing License (LPN)
  • Valid Driver's License
  • Pass a thorough background check
  • Ability to travel to nearby clients as needed
