We are seeking a highly motivated and compassionate Community Health Representative to join our team at NYU Grossman School of Medicine. As a key member of our Section for Health Equity, you will play a critical role in advancing cancer prevention and reducing cancer disparities in immigrant and minority communities.
Key Responsibilities- Cultivate relationships with community-based organizations, faith-based institutions, and community coalitions to increase cancer awareness and promote health education.
- Develop and implement culturally tailored and linguistically appropriate health education materials and survey instruments.
- Provide cancer health education and navigation services to patients, including support with screening, diagnosis, treatment, and survivorship.
- Conduct participant recruitment, consent, survey administration, and data collection for cancer prevention initiatives.
- Identify and address environmental barriers to care, providing referrals for social and supportive services as needed.
- Collaborate with healthcare staff and providers to streamline patient services and referrals.
- Track interventions and outcomes, including data entry into MS Excel, REDCap, and Epic.
- Support administrative duties related to outreach, engagement, and research efforts.
- High School Diploma or equivalent required; Bachelor's degree preferred.
- Minimum 1 year of experience in an office setting or equivalent combination of education and experience.
- Effective oral, written communication, and interpersonal skills.
- Fluency in Russian language preferred.
- Ability to work effectively in a team environment and communicate with all levels of the organization.
