About the Role

We are seeking a skilled Medical Social Worker to join our team at CenterWell Home Health. As a Medical Social Worker, you will play a vital role in providing personalized care to our patients in the comfort of their own homes.

Key Responsibilities
  • Evaluate and develop personalized plans of care to meet the emotional and social needs of our patients
  • Assess relationships between medical and nursing needs, financial resources, and available community resources
  • Provide direction and supervision to Social Worker Assistants as required
  • Collaborate with the interdisciplinary care team to ensure comprehensive care
  • Participate in discharge planning and care coordination activities
  • Supervise, instruct, and evaluate the performance of Social Work Assistants
Requirements
  • Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education
  • Valid driver's license, auto insurance, and reliable transportation
  • Proof of current CPR certification
  • Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice
  • Knowledge of and ability to assist with discharge planning needs and obtain community resources
  • Excellent oral and written communication and interpersonal skills
  • Ability to communicate effectively with patients and their family members
