Job Summary:

\

We are seeking a highly motivated and compassionate Licensed Practical Nurse to join our team at UCAN. As a key member of our healthcare team, you will provide medical care coordination services to youth in our community settings.

\

Key Responsibilities:

\

• Train staff and youth on various health care topics, ensuring they have the knowledge and skills needed to manage their health effectively.\
• Keep accurate and complete case notes on youth, documenting their progress and any challenges they face.\
• Assist the team with completing Asthma Action Plans, helping to ensure that youth with asthma receive the best possible care.\
• Collaborate with the development of service plan goals for youth and children with high-risk/chronic health care needs, working closely with families and caregivers to create personalized plans.\
• Educate youth on the importance of attending routine prenatal care and the effects of cigarettes/drugs/alcohol on the unborn fetus, promoting healthy behaviors from the start.\
• Educate youth on all family planning options, highlighting the pros and cons of each to help them make informed decisions about their reproductive health.\
• Review all prescriptions and medication dosages for youth on psychotropic meds, providing education to the team regarding potential side effects and coordinating with placement providers as needed.\
• Coordinate psychiatric and psychological appointments, ensuring that youth receive the mental health support they need to thrive.\
• Assist youth with medication refills, helping to ensure that they have access to the medications they need to manage their health conditions.\
• Administer hearing and vision screenings for youth and their children, identifying any potential issues early on and connecting them with resources for further evaluation and treatment.\
• Plan and implement health management protocols for youth with chronic health problems, working closely with families and caregivers to develop tailored plans.\
• Liaise with community hospitals to coordinate care for youth and young adults, facilitating communication and collaboration between healthcare providers.\
• Promote and assist in the control of communicable diseases through preventive immunization programs, early detection, surveillance, and reporting of contagious diseases.\
Requirements:

\

• State of Illinois LPN licensure is required.\
• Current CPR certification is necessary.\
• Two years' experience in related work is preferred.\
• Proven ability to work effectively with young people is essential.\
• Understanding of trauma-informed practice is important.\
• Effective verbal and written communication skills are required.\
• Ability to work both independently and as part of a high-functioning team is necessary.\
• Experience and ability to use computers for data entry, communication, and report writing is required.\
• A car and valid Illinois driver's license and insurance are required.\
