Job Summary:We are looking for a skilled Registered Nurse Case Manager to manage a defined caseload of patients, providing direct patient care and comprehensive nursing services to individuals in the community.
Responsibilities:- Manage a caseload of patients, providing direct care and comprehensive nursing services.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with healthcare teams to ensure seamless care coordination.
- Conduct regular assessments and evaluations to monitor patient progress.
- Document patient information and care plans in a timely and accurate manner.
- Valid nursing license in the state of NH.
- 2+ years of experience in skilled care, preferably.
- 1+ year of experience in nursing supervision, preferably.
- Recent wound care experience.
- Ability to lift 50 LBS with no restrictions.
- Current CPR certification.
- Reliable transportation.
