About the Role:

As a Hospice On Call RN, you will be responsible for providing palliative care to clients in designated geographic territories. You will work independently, managing your time effectively and making home visits to clients as assigned. Your duties will include:

  • Following a designated care plan in accordance with patient needs
  • Performing assigned duties, including administration of medication, wound care, treatments, and procedures
  • Monitoring clients' conditions and reporting changes to Clinical or Client Services Manager
  • Following up with, executing, and properly documenting doctors' orders
  • Performing client assessments as necessary
  • Case management and coordination
Requirements:

To be successful in this role, you will need:

  • A current license as a Registered Nurse in Vermont
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience
  • Prior home care experience is strongly preferred, but not required
  • Graduation from an accredited and approved nursing program
  • Ability to work independently and manage time effectively
  • Strong interpersonal skills
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred
