We are seeking a compassionate and skilled Hospice Care RN to join our team at SSM Health. As a Hospice Care RN, you will play a vital role in providing high-quality patient care and support to individuals and their families in their homes and communities.
Job Responsibilities:- Assess and coordinate patient care needs with the healthcare team.
- Perform point-of-care testing and provide skilled nursing care within the established plan of care.
- Collaborate with patients, families, and healthcare teams to develop comprehensive care plans.
- Document visit notes, treatments, and care in a timely and accurate manner.
- Respond to patient and family needs, addressing concerns and issues in a timely and compassionate manner.
- Graduate of an accredited school of nursing.
- Current RN licensure in the state of employment.
- Basic Life Support (BLS) certification.
- Valid driver's license and reliable transportation.
This is a per diem position, working variable hours to meet patient needs. You will be required to travel to patient homes, community-based facilities, and other locations to provide care.
