Physical Therapist - Home Visits:

This is a unique opportunity for a Physical Therapist to work in a home visit setting, providing flexible and supportive care to patients in need. As a member of our team, you will have the chance to make a real difference in the lives of our patients and their families.

About the Role:

As a Physical Therapist in this role, you will be responsible for evaluating and treating patients in their homes, following physicians' orders and developing personalized treatment plans. You will work closely with our team to ensure that patients receive the highest level of care and support.

Responsibilities:

• Evaluate patients using diagnostic tests and measures to assess their condition and develop a treatment plan
• Provide physical therapy services to patients in their homes, following physicians' orders
• Develop and implement personalized treatment plans to help patients achieve their goals
• Document clinical notes and keep records up-to-date
• Work closely with our team to ensure that patients receive the highest level of care and support

Requirements:

• Graduate of an accredited Physical Therapy program
• Current, valid Physical Therapy license issued by the NY Department of Education
• One year of practical experience as a licensed physical therapist
