SALMON Health and Retirement is seeking dedicated and compassionate Licensed Practical Nurses to provide care to patients in their home. As an integral part of our inter-disciplinary care team, you will make home visits to patients living in designated territories, perform assigned duties, and teach and educate patients and/or caregivers on disease process, medications, diet, wound care and other treatments as ordered by the physician. You will also communicate regularly with the RN Case Manager and the patient inter-disciplinary care team to ensure the most appropriate care is being provided. Completing and submitting accurate, timely clinical documentation pertaining to patient interventions and care provided is also a key responsibility.   • A current license as a Licensed Practice Nurse in the state of Massachusetts.
  • Minimum of two years' experience in delivering nursing care.
  • Minimum of one year of Home Health experience preferred.
