Position Overview

Salud Para La Gente (SALUD) is dedicated to delivering exceptional, comprehensive, and affordable primary healthcare services to underserved communities in the Monterey Bay region, including Santa Cruz and Monterey Counties. We are in search of a motivated Care Team Nurse to enhance our clinical care team by contributing to patient care delivery and providing direct nursing services. The Care Team Nurse will employ nursing expertise and judgment to assess patients, deliver education, and perform essential medical procedures, including immunizations and medication administration. Additionally, this role involves collaborating with the Associate Director of Nursing to oversee and mentor certified medical assistants within the care team. The ideal candidate will exhibit strong critical thinking abilities and leadership skills while promoting a team-oriented approach to patient care.

Key Responsibilities:

  1. Implement the nursing process (assessment, planning, intervention, and evaluation) in patient care and daily operations, utilizing evidence-based guidelines and critical analysis of patient information.
  2. Provide independent patient care under established protocols and procedures.
  3. Administer medications and immunizations in accordance with clinical directives.
  4. Deliver direct patient education, clarifying medical procedures, treatments, and medications to patients and their families, while supervising educational efforts by certified medical assistants.
  5. Collaborate with the Associate Director of Nursing to oversee clinical activities performed by certified medical assistants and provide training as needed.
  6. Conduct independent triage assessments via telephone or in-person using established protocols.
  7. Communicate test results and treatment recommendations to patients under written orders.
  8. Ensure efficient patient flow by continually assessing clinician schedules and coordinating with staff to reduce wait times.
  9. Act as Clinic Supervisor when designated.
  10. Engage in staff meetings and contribute to team discussions.
  11. Maintain confidentiality and ensure adherence to privacy policies among certified medical assistants and support staff.
  12. Exhibit exemplary communication skills, utilizing welcoming language, appropriate tone, and positive body language in all interactions with patients and colleagues.
  13. Perform additional duties as assigned.

Qualifications:

  • Current California RN license in good standing.
  • Completion of an accredited RN program.
  • A minimum of one year of experience in acute care nursing.
  • Experience in an ambulatory clinical setting or Federally Qualified Health Center is highly preferred.
  • New nursing graduates with prior supervisory experience are also considered.
  • Valid California Driver's License and current vehicle insurance; ability to travel between sites is required.
  • Bilingual proficiency in English and Spanish is strongly preferred.
  • Current CPR certification.

Physical Requirements:

  • Ability to stand, walk, sit, type, reach, and lift up to 50 pounds.

Compensation and Benefits:

Salary: $122,964.00 annually, with a comprehensive benefits package that includes medical, dental, vision, and life insurance, voluntary life insurance, long-term disability, 401(k) with matching contributions, flexible spending accounts, paid time off, and holiday pay.
