Patient Advocate Position Description

Petaluma Health Center is seeking a skilled Patient Advocate to join our primary care team. As a Patient Advocate, you will play a vital role in ensuring that patients receive timely and appropriate care between visits with their primary care providers.

Key Responsibilities:

  • Collaborate with primary care teams to provide timely review of labs, diagnostic imaging, and documents for patients.
  • Communicate diagnostic results and any follow-up needed subsequently to patients and primary care providers.
  • Serve as backup for urgent questions or review of results if primary care providers are unavailable and patients need assistance for coordination of care.
  • Assist with urgent medication review or refill requests.
Required Skills and Experience:
  • Completion of a nationally accredited Nurse Practitioner or Physician Assistant Program.
  • Two years' experience as a licensed NP or PA provider delivering primary care in a family practice setting, or graduation from a one-year NP residency program.
  • Experience working in a NCQA Patient-Centered Medical Home highly preferred.
  • Experience working in a Joint Commission Accredited health care organization highly preferred.
Salary Range:

$120,000 to $173,806 per year, depending on experience and qualifications.
