This is a full-time opportunity for a compassionate and organized nursing professional to coordinate clinical services between hospital settings and home healthcare or hospice services.

About the Role:
In this rewarding role, you will be responsible for promoting, educating, and coordinating services for patients transitioning from hospital care to home-based care. Your expertise as a registered nurse will be essential in ensuring seamless transitions and providing top-notch patient care in a patient-centric environment.

Key Responsibilities:
  • Coordinate clinical services between hospital settings and home healthcare or hospice services.
  • Promote and educate patients on their care plans to ensure they receive the best possible nursing care.

Requirements:
To succeed in this role, you should have a strong background in nursing with experience in home health/hospice care. A bachelor's degree in nursing is preferred, and CCNE or NLN accreditation is strongly recommended. You must also possess sound knowledge of community resources and hold a valid driver's license. Integrated Care Management Model Certification is an added advantage.
