About the Role:We are seeking a skilled and compassionate Registered Nurse Case Manager to join our team. As a Home Health Registered Nurse Case Manager, you will be responsible for providing high-quality patient care and managing the patient care services related to the curative, rehabilitative, and/or preventive aspects of nursing.
Responsibilities:- Provides skilled services to patients at home and manages the patient care services pertaining to the curative, rehabilitative, and/or preventive aspects of nursing.
- Makes the initial evaluation visit and regularly re-evaluates the patient's nursing needs.
- Initiates the nursing plan of care with appropriate preventive and rehabilitative nursing procedures and necessary revisions.
- Furnishes those services requiring substantial and specialized nursing skill.
- Prepares clinical and progress notes the day of service.
- Coordinates services and informs the physician and other personnel of changes in the patient's condition and needs.
- Counsels the patient and family in meeting nursing and related needs.
- Participates in in-service programs and supervises and teaches other nursing personnel.
- May delegate aspects of patient care to unlicensed individuals.
- Teaches and trains patients and caregivers nursing care.
- Provides nail care to patients and performs medication management setup.
- Graduate from an accredited nursing school.
- Requires current nursing license in the state in which you are applying for work in, current CPR certification, current driver license in the state in which you are applying for work in or transportation available to cover entire service area without advance notice, and current minimum automobile insurance coverage as required by law.
- Interest in working with ill, elderly and/or disabled patients, with at least one year of recent experience in a clinical setting.
- Competitive hourly rate of $45.00 to $53.00 per hour, plus mileage and benefits.
- Flexible work schedules, including PRN, Part-Time or Full-Time, 10-hour shifts available, 30-to-40-hour work week, benefits offered for Part-Time and Full-Time employment.
