We are seeking a highly skilled and compassionate Registered Nurse to join our team at Park Gardens Rehabilitation and Nursing Center. As a Registered Nurse, you will play a vital role in providing high-quality patient care and contributing to the overall success of our facility.
Key Responsibilities- Provide direct nursing care to residents under the medical direction and supervision of attending physicians, the Director of Nursing Services, and the Medical Director of the facility.
- Monitor the performance of non-licensed personnel and assist in modifying treatment regimens to meet the physical and psychological needs of residents.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that promote resident independence and well-being.
- Conduct regular assessments and evaluations to identify resident needs and develop strategies to address them.
- Communicate effectively with residents, families, and healthcare teams to ensure seamless care coordination and transitions.
- Participate in ongoing education and training to stay current with best practices and regulatory requirements.
- Graduate from an accredited school of nursing.
- Current, unencumbered, active license to practice as a RN in New York.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to lift and move patients and equipment as needed.
- Ability to work in a fast-paced environment and adapt to changing situations.
- Previous long-term care experience.
- Computer knowledge and proficiency.
- Ability to speak and write the English language in an understandable manner.
- Works throughout the nursing service area.
- May be subject to frequent interruptions and hostile or emotionally upset residents, family members, and visitors.
- May be required to work beyond normal working hours and in other positions temporarily.
- May be subject to callback during emergency conditions.
