HHC is seeking a dedicated Licensed Practical Nurse to join our Ambulatory Care team at Harlem Hospital Center. Our team provides a wide range of medical, surgical, diagnostic, therapeutic, and family support services to the residents of Central Harlem, West Harlem, Washington Heights, and Inwood.

About the Role:

  1. Promote a positive and pleasant attitude conducive to the workplace which enhances the Mission, Vision, and Value Statement of the organization.
  2. Manage chronic disease/population health initiatives and work toward meeting the HHC guidelines.
  3. Initiate and complete patient admission history and assessment with clear communication and pertinent documentation in accordance with established guidelines.
  4. Assess, screen, triage, and delegate assignments for nursing personnel based on patient acuity and employee's skill level.
  5. Perform a systematic assessment utilizing Evidence-based Practice including physiological and psychosocial needs. Prioritize and initiate, implement, and evaluate a patient-centered nursing care plan based on a pathophysiologic knowledge and patient assessment. Evaluate and revise care plans in a regular basis and establish and monitor a realistic short- and long-term goals with patient and/or families.
  6. Explain rationale to patients for selected therapies based on observation and findings and participate in patient advocacy - compassion, caring, and emotional support.
  7. Evaluate the patient's response to therapeutic regimen and outcome management and communicate with providers.
  8. Explain, educate, and instruct, and manage patient/family concerning on therapeutic regimen and environment of care in collaboration with medical provider.
  9. Collaborate with other health professionals in planning care for the patients.
  10. Participate and delegate functions effectively, both individually and as team member, during crisis and
  11. Demonstrate the knowledge of nursing unit and departmental standard, institutional policy requirement in accordance with Regulatory Compliance (OSHA, JACHO).
  12. Accept and demonstrate responsibility to co-workers, unit, and institution and Accountability.
  13. Develop knowledge in the principles of change theory and support group consensus regarding change.
  14. Keep abreast of current modalities, and provide evidence-based practice to reinforce Primary, Secondary, and Tertiary prevention.
  15. Update knowledge to current nursing practices, and seek additional training in new techniques, information, and services.
  16. Perform proficiently in medication management including obtaining meds, storage, par level, and administration of medications.
  17. Perform venipuncture when necessary.
  18. Perform point of care testing, e.g. blood glucose. Supervise ancillary staff providing POCT services.
  19. Obtain 12-Lead EKG when appropriate.
  20. Perform dressing changes, e.g. wound and assist in surgical and other procedures.
  21. Perform proficiently in discharging patients post visit including discharge education/instructions
  22. Participate in emergency code, e.g. Condition Help, CAC and records activity.
  23. Initiate emergency administration of oxygen according to policy and procedure.
  24. Conform to policies governing patient abuse, patient rights, and confidentiality of information.
  25. Meet all compliance standards/requirements such as Annual education & Evaluations, and Customer Service Training.
  26. Enhance personal and professional development through attendance to internal in-service training and external educational offerings.
  27. Responsible to practice ICARE actionable Behaviors.

Requirements:

  • Currently licensed or otherwise duly authorized to practice as a Licensed Practical Nurse in the State of New York.
