Position Overview

Mountain Empire Older Citizens Inc is seeking a dedicated Licensed Practical Nurse (LPN) to provide essential nursing care to our participants within the PACE program. This role involves working under the supervision of registered nurses and primary care providers to ensure the highest quality of care.

Key Responsibilities

The LPN will be responsible for:

  • Coordinating medical appointments for participants, addressing both urgent and routine needs.
  • Conducting thorough nursing assessments to identify the healthcare needs of participants.
  • Monitoring and documenting patient data accurately in medical records.
  • Recognizing and reporting any issues that require further evaluation or intervention.
  • Adhering to established care plans and maintaining detailed records of participant progress.
  • Providing wound care in both clinical and home settings as directed by medical orders.
  • Assisting participants with medication management and organization.
  • Collecting laboratory specimens in accordance with clinical protocols.
  • Administering EKG tests and medications as prescribed.
  • Making home visits as necessary, based on medical directives or urgent needs.
  • Supporting participants with personal care, including foot and nail care.
  • Gathering information regarding participant falls and documenting appropriately.
  • Educating participants and their caregivers about medical conditions, treatments, and medications.
  • Maintaining clinical logs for various health metrics and supplies.
  • Ensuring nursing care is delivered in line with accepted professional standards.
  • Participating in inventory management of medical supplies and reporting needs to supervisors.
  • Engaging in quality assurance and performance improvement initiatives.
  • Documenting any changes in participant health status and communicating these to the care team.
  • Collaborating effectively with home care staff, transportation personnel, and other service providers.
  • Identifying emergency situations and responding appropriately.
  • Upholding participant rights and delivering care in an ethical manner.
  • Contributing to the coordination of comprehensive care delivery.
  • Managing after-hours care needs, including triaging situations for appropriate responses.
  • Completing all required documentation in a timely manner.
  • Performing duties within the scope of practice.
