About the Role

We are seeking a skilled Registered Nurse to join our Home Care team in Fridley. As a Home Health RN, you will provide high-quality patient care and support to individuals in their homes, promoting independence and well-being.

Key Responsibilities
  • Conduct comprehensive assessments of patients and families to identify their needs and develop individualized care plans.
  • Provide nursing care and interventions to patients with various health conditions, including rehabilitation, medical intervention, and end-of-life care.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and continuity.
  • Supervise and delegate tasks to other healthcare professionals as needed.
  • Participate in ongoing education and professional development to stay current with best practices and evidence-based care.
Requirements
  • Associate's or Vocational degree in Nursing
  • 0-2 years of RN experience, with preference for homecare/hospice experience
  • Licensed Registered Nurse - MN Board of Nursing required
  • BLS Tier 1 - Basic Life Support - Multisource required
