AccentCare is seeking a highly skilled and experienced Registered Nurse (RN) to fill the role of Home Health Clinical Manager. As a key member of our team, you will be responsible for overseeing the day-to-day clinical and office operations, ensuring that our services are delivered in compliance with professional standards, agency policy, and state and federal regulatory requirements.
Key Responsibilities:- Supervise and coordinate referrals, patient care, and interdisciplinary team approaches.
- Develop, implement, and update individualized plans of care, ensuring that patient needs are continually assessed and met.
- Collaborate with clinicians and physicians to ensure seamless patient transfers and discharges.
- Manage KPIs, workflow, escalation/complaint calls, and guide professional clinicians on implementing Emergency Care Protocols.
- Ensure clinicians, including contract staff, have required training, competency, and supervision.
- Participate in QA and performance improvement activities, reviewing and investigating incidents, and implementing corrective action plans as needed.
- Registered nurse with current licensure to practice nursing in the practicing state.
- Possess and maintain CPR certification.
- Previous experience in home care setting with two years management or supervisory experience, preferred.
- Proven ability to work within an interdisciplinary setting.
- Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and is in good working order.
