AHN Healthcare@Home is seeking a compassionate and skilled Registered Nurse to join our team of healthcare professionals. As a Home Health RN, you will play a vital role in delivering high-quality patient care in the comfort of patients' homes.

About the Role:

  • Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and develop plans to address these needs.
  • Administer medications, perform treatments, and provide education to patients and families.
  • Document patient progress and communicate with healthcare teams to ensure seamless care.

Requirements:

  • RN license in the state of employment.
  • One year of RN experience and the clinical competence to deliver quality patient care.
  • Current driver's license and ability to spend ~20% of your day driving to/from patient locations.
  • Demonstrated experience with tablets, mobile phones, and EMR software.
