APIP Case Manager

Join the Fortune Society Inc. team as an APIP Case Manager and play a vital role in facilitating Abusive Partner Intervention groups and providing case management services to clients.
Job Summary:The APIP Case Manager will be responsible for facilitating Abusive Partner Intervention groups, performing general case management duties, and providing referrals to community-based services. The ideal candidate will have experience working with mandated participants with criminal justice involvement and possess strong assessment and engagement skills.
Key Responsibilities:- Facilitate Abusive Partner Intervention groups using evidence-based curricula
- Perform general case management duties, including weekly individual sessions with clients, referrals to community-based services, and developing client-centered treatment plans
- Assist clients in identifying and prioritizing needs
- Provide referrals to ensure all participants' concrete needs are addressed
- Ensure accurate and timely data entry of all client information into the agency's database
- Provide accurate and timely data and information to be included in court reports
- Monitor participants' attendance and compliance with mandate requirements
- Conduct toxicology for clients as needed
- Participate in case conferences for optimum collaboration among departments/programs and external agencies
- Attend and participate in meetings, conferences, and trainings
- Conduct weekly chart audits
- Engage in ongoing professional development
- High school diploma or GED/HSE required, some college preferred
- Experience and commitment to working with mandated participants with criminal justice involvement
- Strong Microsoft Office and database entry skills
- Strong assessment and engagement skills
- Strong written and verbal communication skills
- Bilingual English/Spanish a plus
- Strong organizational and leadership skills
