Key Responsibilities
  • Assist patients with personal care tasks as outlined in their care plan, including hygiene, mobility, and elimination needs.
  • Ensure a safe environment for patients by using equipment properly and safely.
  • Adhere to Universal Infection Control Precautions during all patient interactions.
  • Support patients and families with home maintenance tasks as specified in the care plan, such as light cleaning and meal preparation.
  • Monitor and report on patients' fluid and nutrition intake, vital signs, and any changes in their condition.
  • Utilize safe transfer and positioning techniques to assist patients.
  • Identify the need for additional interventions from the healthcare team and communicate with the RN or Clinical Services Director.
  • Respect patient confidentiality and autonomy at all times.
  • Document care provided and patient responses accurately and promptly.
  • Operate within the scope of your training and experience.
  • Comply with all relevant laws, regulations, and agency policies.
  • Manage a caseload effectively while delivering cost-efficient care.
  • Consistently meet or exceed the service standards set by 365 Health Services.
  • Conduct all professional activities ethically and responsibly.
  • Utilize supplies and equipment in an effective manner.
  • Complete a minimum of 12 hours of training annually.
  • Maintain regular attendance and punctuality.
  • Adhere to the Code of Conduct established by 365 Health Services.
  • Foster positive interactions with all staff, supporting the mission and vision of the organization.
  • Follow agency policies and procedures, including confidentiality and HIPAA guidelines.

Starting Salary: $15.00 per hour
