Job SummaryWe are looking for an experienced Speech Language Pathologist to provide speech and language services to adult and geriatric patients in the comfort of their own homes. As a member of our Home Health team, you will be part of a multidisciplinary team that provides rehabilitative care and skilled nursing to clients, allowing them to receive the medical care required to remain at home.
Responsibilities- Conduct thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients who are experiencing speech, language, communication, or swallowing difficulties.
- Develop treatment plans based on evaluation results, physician's orders, and recommended goals.
- Continuously assess and revise speech therapy care plans, participating with BAYADA clinicians in the multidisciplinary care plan as appropriate.
- Educate and instruct patients, family members, or other patient representatives in rehabilitative care and activities necessary to promote client health, safety, and independent living.
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
- Current license as a Speech Language Pathologist in MD.
- Certificate of clinical competence in speech from the American Speech and Hearing Association, or equivalent educational requirements and work experience.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
