Job Title: Home Health Registered Nurse

Interim HealthCare of Oakland County is seeking a skilled and compassionate Home Health Registered Nurse to join our team. As a Home Health Registered Nurse, you will be responsible for providing high-quality patient care and services to clients in their homes.

Key Responsibilities:

  • Conduct in-home assessments and create personalized care plans to meet the unique needs of each client.
  • Work collaboratively with the interdisciplinary team to achieve home health goals and ensure quality patient care.
  • Make decisions based on your educational preparation and experience in nursing, serving as the case manager for each client's care plan.
  • Provide education and support to patients and their families to promote healthy outcomes and independence.

Requirements:

  • Graduate of an accredited school of nursing with a current license as a Registered Nurse in the state(s) in which you practice.
  • Two years of current nursing experience, with a preference for case management experience and community health experience.
