Work Schedule:
Monday: 9 AM - 1 PM
Wednesday: 9 AM - 1 PM
Friday: 9 AM - 1 PM
Key Responsibilities:
- Deliver comprehensive, patient-focused primary care services at the Health and Wellness Center.
- Conduct episodic, preventive, and primary health assessments.
- Collaborate with patients to create tailored treatment plans.
- Perform physical examinations, health-risk evaluations, and thorough medical histories.
- Facilitate referrals to appropriate medical specialists or resources as necessary.
- Contribute to the establishment of annual goals and objectives for the Health and Wellness Center.
- Work collaboratively with a medical assistant, patient advocate, and services director to promote a team-oriented atmosphere.
- Embody and promote the vision, mission, and values of Proactive MD.
Qualifications:
- Minimum of 1 year of post-residency experience.
- Valid and current state medical license.
- Board certification or eligibility in Family Medicine.
- Strong interpersonal communication skills.
- Able to effectively engage with individuals from diverse backgrounds.
- Familiarity with the healthcare sector and available patient resources.
- Understanding of HIPAA confidentiality standards.
- Current BLS or ACLS certification.
