We are seeking a highly skilled Clinical Faculty(Open Rank)/Clinical Pharmacist to join our Antithrombosis team at the University of Illinois Hospital & Clinics. The ideal candidate will provide pharmaceutical care to patients with major disease states requiring anticoagulation therapy.
Key Responsibilities- Provide pharmaceutical care to patients with anticoagulation therapy needs
- Develop and implement treatment plans for patients with anticoagulation therapy
- Collaborate with healthcare teams to ensure patient safety and effective care
- Conduct research and scholarly activities to advance the field of anticoagulation therapy
- Teach and mentor students and residents in the College of Pharmacy
- PharmD degree and PGY1 residency experience required
- Illinois licensure as a professional pharmacist required
- Strong clinical knowledge base and analytical skills
- Excellent communication and interpersonal skills
- PGY2 Ambulatory Care residency or 2-3 years of equivalent clinical experience
- Board certification in a relevant area
