Family Support Specialist

**Job Title:** Family Support Specialist
We are seeking a highly skilled and compassionate Family Support Specialist to join our team at Inclusive Employment Services. As a key member of our support services team, you will play a vital role in assisting individuals and families by coordinating various services and supports that enable participants to maintain their residence within their family home.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work independently and demonstrate problem-solving skills in challenging situations. Strong adherence to confidentiality and ethical guidelines is expected. Proficiency in English and Spanish language (reading, writing, speaking) is highly desirable.
About the Role:
- Develop skills within the home environment to foster independence and growth.
- Conduct initial CFS assessments of participants and families.
- Prepare Individual Service Plans (ISPs) based on reports of discovery, inputs from participants and families.
- Review minutes of meetings between CFS workers and participants to ensure effectiveness of service plans and adjust as needed.
- Administer quarterly performance evaluation surveys on participants and families.
- Help access community resources and services.
- Coordinate delivery of necessary services to meet individual needs.
- Offer information and resources pertinent to individuals' diagnoses and required supports.
- Manage scheduling for medical, educational, vocational, and other appointments as needed.
- Write quarterly reports to program directors on survey results, including remedial actions where necessary.
- Identify transportation options or services for individuals.
- Assist with future planning initiatives.
- Oversee entire delivery of services to ensure effectiveness.
About You:
- Bachelor's degree in a human services field of study or Associates degree in a human services field of study and 3 years of experience working with people with developmental, cognitive, or physical disabilities.
- Excellent communication and interpersonal skills.
- Understanding of mental health and developmental disabilities preferred.
- Strong adherence to confidentiality and ethical guidelines.
- Proficiency in English and Spanish language (reading, writing, speaking) highly desirable.
- Reliable vehicle, valid driver's license, and car insurance required.
- Criminal background check mandatory.
Salary Estimate: $55,000 - $65,000 per year, depending on qualifications and experience.
