ChristianaCare is seeking a highly skilled Registered Nurse (RN) to join our team as a Case Manager in an acute care hospital setting. As a key member of our care management team, you will be responsible for coordinating patient care and driving patient progression to establish a discharge plan.
About the RoleThis is a day shift position, Monday through Friday, with a 8am-4:30pm schedule and a call rotation every 7 weeks. No holidays or weekends are required. The ideal candidate will have a minimum of 2 years of medical surgical nursing experience in an acute care hospital and a strong background in care management.
Responsibilities- Coordinate patient care and develop discharge plans in collaboration with the interdisciplinary team.
- Identify patients with post-acute care needs and develop strategies to remove barriers to care.
- Collaborate with physicians, nurses, and other healthcare providers to ensure a multidisciplinary plan of care is in place.
- Participate in the development and implementation of strategies to improve patient outcomes and reduce readmissions.
