Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Roosevelt Care Centers. As an LPN, you will provide high-quality nursing care to our residents, working under the direction of our Registered Nurses (RNs) and in collaboration with our interdisciplinary team.

Key Responsibilities
  • Provide direct care to residents, including administration of medications, treatments, and other nursing interventions.
  • Monitor residents' conditions and respond appropriately, notifying RNs or Nursing Supervisors as needed.
  • Document resident responses and nursing interventions in medical records.
  • Assist in completion of Minimum Data Set (MDS) and care planning process under RN direction.
  • Orient residents and their families to our long-term care environment and maintain liaison with families.
  • Participate in committees and conferences, and orient and assist other clinical personnel.
  • Initiate referrals to other departments, services, and/or outside agencies for resident care.
  • Provide comprehensive patient/resident care.
  • Consult with supervisors and others regarding significant changes in resident's condition and relay appropriate information to succeeding shift.
  • Ensure medical and emergency equipment/devices are in operating condition and available.
  • Participate in wound care rounds.
  • Assist in the orientation and training of new personnel.
Requirements
  • Graduation from an accredited school of practical nursing and current NJ license to practice as a Licensed Practical Nurse.
  • One year of experience in clinical nursing preferred.
  • Ability to read, write, speak, and understand English sufficiently to perform the job.
